The creator is expected to make sure that the action does not exist. Sometimes, they do not, so make a bit of effort to find out yourself. If there is a similar action, then refuse the new one. On the first offense: Put a comment in "Needs more work" and offer the author to change the action to make it significantly different from other actions. This can e.g. be done by re-phrasing the Descriptions, so that a kink is required. If the same player keeps submitting duplicate actions, then start destroying them without asking and tell them why.
